Beyond serving as passive carriers of molecular cargo, these vesicles function as <strong data-start=\"464\" data-end=\"486\">dynamic modulators<\/strong>, capable of influencing immune activation, tolerance, and homeostasis depending on the physiological and pathological context.<\/p>\n<h3 data-start=\"615\" data-end=\"682\">Immunomodulatory Exosomes: Balancing Activation and Suppression<\/h3>\n<p data-start=\"684\" data-end=\"876\">Various immune cells\u2014<strong data-start=\"705\" data-end=\"772\">dendritic cells, T and B lymphocytes, macrophages, and NK cells<\/strong>\u2014secrete exosomes with <strong data-start=\"795\" data-end=\"828\">specific molecular signatures<\/strong> that determine their immunomodulatory function:<\/p>\n<ul data-start=\"878\" data-end=\"1382\">\n<li data-start=\"878\" data-end=\"1129\">\n<p data-start=\"880\" data-end=\"1129\"><strong data-start=\"880\" data-end=\"926\">Selective activation of CD8\u207a T lymphocytes. <\/strong>Through exosomes derived from dendritic cells presenting antigens and co-stimulatory molecules in a hierarchical manner, enhancing clonal expansion of effector cells and the generation of immune memory.<\/p>\n<\/li>\n<li data-start=\"1130\" data-end=\"1382\">\n<p data-start=\"1132\" data-end=\"1382\"><strong data-start=\"1132\" data-end=\"1168\">Tumor-mediated immunosuppression.<\/strong>  Exosomes carrying <strong data-start=\"1188\" data-end=\"1271\">PD-L1, FasL, TGF-\u03b2, IL-10, and regulatory microRNAs (miR-21, miR-155, miR-146a)<\/strong> inhibit NK and cytotoxic T cells, promoting peripheral tolerance and remodeling of the tumor microenvironment.<\/p>\n<\/li>\n<\/ul>\n<p data-start=\"1384\" data-end=\"1550\">These mechanisms highlight the <strong data-start=\"1415\" data-end=\"1452\">functional plasticity of exosomes<\/strong>, capable of acting both locally and systemically, precisely and coordinately modulating immunity.<\/p>\n<h3 data-end=\"1712\">Induce Treg expansion <\/strong>by transporting immunosuppressive molecules and specific miRNAs that regulate NF-\u03baB and STAT3 signaling pathways.<\/p>\n<\/li>\n<li data-start=\"1827\" data-end=\"2021\">\n<p data-start=\"1830\" data-end=\"2021\"><strong data-start=\"1830\" data-end=\"1875\">Reprogram dendritic cells and macrophages<\/strong> towards tolerogenic phenotypes, reducing the expression of co-stimulatory molecules (CD80\/CD86) and the production of pro-inflammatory cytokines.<\/p>\n<\/li>\n<li data-start=\"2022\" data-end=\"2147\">\n<p data-start=\"2025\" data-end=\"2147\"><strong data-start=\"2025\" data-end=\"2063\">Modulate the immunological synapse<\/strong> by interfering with <a href=\"https:\/\/www.cell.com\/fulltext\/S0092-8674(01)00185-4\">TCR-MHC<\/a> complex formation and limiting T lymphocyte activation.<\/p>\n<\/li>\n<\/ol>\n<p data-start=\"2149\" in oncology, autoimmunity, and transplantation.<\/p>\n<h3 data-start=\"2329\" data-end=\"2381\">Exosomes as Platforms for Advanced Immunotherapy<\/h3>\n<p data-start=\"2383\" data-end=\"2498\">Understanding these mechanisms is paving the way for the development of <strong data-start=\"2455\" data-end=\"2497\">next-generation therapeutic strategies<\/strong>:<\/p>\n<ul data-start=\"2648\" data-end=\"2791\"><strong data-start=\"2648\" data-end=\"2716\">Exosomes engineered to inhibit PD-L1 or deliver immunostimulants<\/strong>, enhancing T lymphocyte activation in immunosuppressive microenvironments.<\/p>\n<\/li>\n<li data-start=\"2792\" data-end=\"2932\">\n<p data-start=\"2794\" data-end=\"2932\"><strong data-start=\"2794\" data-end=\"2839\">Delivery vehicles for bioactive molecules<\/strong> via synthetic or hybrid exosomes, optimizing bioavailability and reducing systemic toxicity.<\/p>\n<\/li>\n<\/ul>\n<p data-start=\"2934\" data-end=\"3115\">These strategies position exosomes as <strong data-start=\"2972\" data-end=\"3035\">biocompatible, precise, and modulable therapeutic platforms<\/strong>, with potential to integrate personalized immunotherapy and precision medicine.<\/p>\n<h3 data-start=\"3117\" data-end=\"3140\">Future Perspectives<\/h3>\n<p data-start=\"3142\" data-end=\"3670\">The development of <strong data-start=\"118\" data-end=\"154\">high-purity isolation techniques<\/strong>, <strong data-start=\"156\" data-end=\"193\">multi-parametric characterization<\/strong>, and <strong data-start=\"199\" data-end=\"231\">advanced functional analysis<\/strong> will deepen our understanding. It will specifically enhance knowledge of the <strong data-start=\"311\" data-end=\"354\">immunomodulatory mechanisms of exosomes<\/strong>. These approaches will also enable their use as <strong data-start=\"401\" data-end=\"419\">clinical tools<\/strong> for designing <strong data-start=\"434\" data-end=\"447\">therapies<\/strong> and <strong data-start=\"452\" data-end=\"480\">immunological biomarkers<\/strong>. The combination of <strong data-start=\"503\" data-end=\"529\">EV molecular profiling<\/strong>, <strong data-start=\"531\" data-end=\"550\">genetic editing<\/strong>, and <strong data-start=\"556\" data-end=\"591\">high-performance bioinformatics<\/strong> opens the door to highly specific <strong data-start=\"626\" data-end=\"655\">therapeutic interventions<\/strong>.
